I HAVE READ AND UNDERSTAND all terms and conditions of employment that must adhered to as contained in the Tournament Caddie Regulations. I hereby agree to abide by the Tournament Caddie Regulations and I acknowledge that in not doing so, I will not be permitted to work at this Tournament and further that the Canadian Tour may prohibit me from being a caddie at any other Canadian Tour event. In addition, and in accordance with the Canadian Tour Rules and Regulations, my conduct may result in the Canadian Tour imposing a fine or in taking other disciplinary action against my player.
